How can I file a Dispute?

Written Dispute: If the Member is unable to resolve an issue to their satisfaction by calling OneShare Health, the Member may file a written dispute within 60 days after the act or decision giving rise to their dispute.

Disputes may be submitted in three ways:
  • By completing OneShare Health’s Member Dispute Form and submitting it to Dispute@onesharehealth.com.
  • By completing the Dispute Form and mailing it to OneShare Health, Attn: Disputes, P.O. BOX 825, Uniontown, OH 44685.
  • By going online to
www.onesharehealth.com/memberdisputes  and downloading and submitting the form per the instructions above.
